On Monday, January 11, 1993, at about 10:00 a.m. a multiple fatal roof fall accident occurred in the 002 Mains section of JEN, Inc.'s No. 30 Mine, resulting in the deaths of the mine foreman and the roof bolting machine operator. The scoop-car operator and the timberman were injured as a result of the accident.

Fourteen miners were present on the working section at the time of the accident. Four of the miners died instantly as a result of the massive roof fall."
