This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

The Second World War was the defining conflict of the twentieth century and it is one of the most popular and fascinating areas for historical research and for family historians. More records than ever are available to researchers whose relatives served during the war. And this new book by Phil Tomaselli is the perfect guide to how to locate and understand these sources and get the most out of them. He explains how, and from where, service records can be obtained, using real examples showing what they look like and how to interpret them. He also examines records of the military units relatives might have served in so their careers can be followed in graphic detail. The three armed services are covered, along with the merchant navy, the Home Guard, civilian services, prisoners of war, gallantry and campaign medals, casualties, womens services and obscure wartime organizations. Also included are a glossary of service acronyms, information on useful websites, an introduction to the National Archives and details of other useful sources.

This eye-opening account reveals that covert intelligence operations in the U.S. date much farther back than most people realize--back to the Founding Fathers. Detailing clandestine, unscrupulous operations that took place under such presidents as Washington, Jefferson, Polk, and Lincoln, Knott reveals that presidents have rarely consulted Congress before engaging in such operations.

The first edition of this book, published in 1999, was well-received, but interest in it has surged in recent years. It chronicles an early example of “regime change” that was based on a flawed interpretation of intelligence and proclaimed a success even as its mistakes were becoming clear. Since 1999, a number of documents relating to the CIA’s activities in Guatemala have been declassified, and a truth and reconciliation process has unearthed other reports, speeches, and writings that shed more light on the role of the United States. For this edition, the author has selected and annotated twenty-one documents for a new documentary Appendix, including President Clinton’s apology to the people of Guatemala.
