It's two o'clock in the morning. Your family has gathered in the emergency room, awaiting information on a loved one. Your heart rate is elevated; you feel overwhelmed. The anxiety you're experiencing will likely increase in the coming hours, days, and months.Medical emergencies disrupt communications and relationships, even in the most stable of families. Who is responsible for communicating with medical staff, often from different specialties? Who will monitor the patient's daily care and progress? Who decides where the patient will go after hospitalization? Who feeds the pets and pays the bills at home? This handbook is a road map to guide families during this unfamiliar and stressful time. It covers twenty years of navigating the healthcare system as consumers and working in it as practitioners. The book provides a step-by-step chronological list of questions to ask and decisions to make, from the first emergency room visit through aftercare options. It advises on numerous topics, including scheduling surgery, monitoring prescriptions, selecting a short or long-term care facility, assigning friends and family helpful tasks beyond the casserole, and how best to advocate for your patient throughout the process.

Recent research underscores a serious lack of preparedness among hospitals nationwide and a dearth of credible educational programs and resources on hospital emergency preparedness. As the only resource of its kind, Health Care Emergency Management: Principles and Practice specifically addresses hospital and health system preparedness in the face of a large scale disaster or other emergency. During medical emergencies, hospital staff and emergency medical services (EMS) providers, can face barriers in delivering the fastest and best possible care. Overcrowded emergency rooms cannot care for patients as quickly as necessary, and some may divert ambulances and turn away new patients outright. In many states, ambulance staff lacks the means to determine which hospitals can provide the best care to a patient. Given this absence of knowledge, they bring patients to the closest hospital. In addition, because emergency service providers from different companies compete with each other for patients, and emergency care legislation varies from state to state, it is difficult to establish the necessary local, interstate, and national communication and collaboration to create a more efficient system. In 2006, the IOM recommended that the federal government implement a regionalized emergency care system to improve cooperation and overcome these challenges. In a regionalized system, local hospitals and EMS providers would coordinate their efforts so that patients would be brought to hospitals based on the hospitals' capacity and expertise to best meet patients' needs. In September 2009, three years after making these recommendations, the IOM held a workshop sponsored by the federal Emergency Care Coordination Center to assess the nation's progress toward regionalizing emergency care. The workshop brought together policymakers and stakeholders, including nurses, EMS personnel, hospital administrators, and others involved in emergency care. Participants identified successes and shortcomings in previous regionalization efforts; examined the many factors involved in successfully implementing regionalization; and discussed future challenges to regionalizing emergency care. This document summarizes the workshop.

Many primary care clinicians are busy to the point of being overwhelmed and there is not time to do thorough evaluations on every patient. The outpatient clinician must be able to quickly identify and manage the overtly ill but also those with potential or even hidden issues that require a referral to the emergency department. This book provides a concise yet comprehensive summary of the various conditions the primary care practitioner could encounter in his/her office that require emergency department referral. Organized system by system, this manual provides short, readable yet detailed descriptions of situations where the clinician must make the quick decision to escalate the level of care. The book is comprised of thirteen main sections, each detailing a medical specialty and is further broken down into specific conditions within these specialties. The specialties that are featured are cardiology, dermatology, endocrinology, gastroenterology, HEENT, hematology and oncology, infectious disease, nephrology, neurology, gynecology, ophthalmology, psychiatry, pulmonology and urology. The chapters are broken down into five key aspects. First, a brief synopsis of the problem is identified covering the incidence, the risk factors and general information to give the clinician the insight to manage the situation. Next, there is a review of key elements of the history that can elucidate whether the patient has or does not have the emergent condition. Physical exam findings that the clinician can look for during the evaluation are then discussed followed by diagnostics that the clinician can get quickly the same day to help evaluate the situation. Lastly, treatments that can be provided while waiting for the urgent or emergent condition to be evaluated are identified. Written by experts in the field, Managing Emergencies in the Outpatient Setting is a valuable resource for primary care physicians, physician assistants and nurse practitioners alike.

Bioethics and the Law takes a multidisciplinary approach that combines legal discussion with jurisprudential, philosophical, and sociological materials. Strong expressions of different points of view highlight debates about bioethical issues. The text underscores the need to mediate between the law's focus on broad rules and the bioethicist's concern with context and detail. Students are required to consider the ethical implications of health care as a business, face the shifting parameters of the provider/patient relationship in healthcare, and understand the role of government in designing and implementing healthcare programs such as Medicaid and Medicare. Bioethics and the Law supplements the traditional focus of bioethics on the interest of the individual with a second focus on the socio-economic developments that shape healthcare. Connecting broad public healthcare issues to concerns of the individual patient/healthcare consumer, the text promotes understanding of unsettling and complex situations and shows the implications of bioethical developments for understandings of personhood. A helpful glossary defines basic terms and several short appendices summarize recent developments in science and technology.
