Platelet-activating factor (PAF) is the commonly used name for a group of potent biologically active, ether-linked phospholipids, the alkylacetylglycerophospho cholines. Following the identification of the chemical structure of P AF in 1979, much progress has been made in our understanding of these phospholipid medi ators. This is particularly true from the points of view of their molecular structural requirements for expression of biological activity, their biosynthesis and cata bolism, their diverse range of biological activities, evidence for their specific recep tors at the cell surface, the development of specific antagonists for use in receptor studies and for testing as potential anti-PAF drugs, and implications about their involvement in health and disease. Chapters in this book cover the current status of these general areas of PAF research. Although an enormous amount of information has been published about this interesting new type of phospholipid mediators, questions about their mechanism of action, regulatory controls, and the precise role and relative importance of P AF in specific diseases and physiological functions remain to be answered. Since the 1979 discovery, the PAF field has brought together scientists from a variety of different biomedical disciplines. The diversity of scientists now in PAF research is very apparent from the number of P AF publications appearing in a wide array of scientific journals, ranging from molecular biology to clinical medicine. Publications related to PAF continue to increase at an amazingly rapid rate, with the pace reminiscent of the period following the discovery of prostaglandins.

In the last decade, research on platelet-activating factor (PAF) has expanded exponentially. Previous conferences on PAF in Paris, 1983, and the subsequent conferences in Gatlinburg, Ten nessee, Tokyo, Snowbird, Utah, and Berlin, at three-yearly intervals, have chronicled the devel opments in the field ofPAF. This volume records the proceedings of the Fifth International Con gress on PAF and Related Lipid Mediators, held at the Free University Medical Hospital Ben jamin Franklin in Berlin, from September 12-16, 1995. We are very much indebted to Free Uni versity Berlin for providing tremendous facilities and financial support. It was a great pleasure to have positive and generous input from the German Science Council (DFG), Bonn, Germany, and British·Biotech, Oxford, United Kingdom. Recently, there has been tremendous scientific interest in the role of phospholipids and particularly phosphatidylcholine (lecithin) in a variety of biological processes. These include the involvement of phosphatidylcholine in biological membranes, as a component of plasma lipoprotein, as a transporter of choline in the body, and also as a "reserve", and possibly only source of unesterified choline. Moreover, numerous clinical studies have recently been conducted, to evaluate the possible uses of externally supplied lecithin in the treatment of some intractible neuropsychiatric disorders (e.g. tardive dyski.nesia, Alzheimer's disease, etc.) and other conditions (e.g. hypercholesteremia). Results to date are encouraging, yet equivocal. This is due, in part, to the fact that the field of phospholipid methodology is highly complex. There is much confusion in the literature, and many ambiguities still remain in the interpretation of experimental findings. This is particularly so for phenomena involving phospholipid function in the central nervous system. This book incorporates the proceedings of the Fourth International Colloquium on Lecithin, which took place in Chicago, Illinois, USA, on September 15-17, 1986. The purpese of this colloquium was to review, in a comprehensive manner, basic principles as weli as current information about the technology, biochemistry, physiology and therapeutic potential of lecithin. Over 108 individuals from all over the world participated in the sessions. The meeting was subdivided into oral presentations and panel discussions (see Table of Contents). It was sponsored and financed by the Lucas Meyer GmbH (Hamburg), for which the editors are most grateful.

In recent decades eicosanoids have been attracting an increasing amount of attention as a result of their important physiological roles in many areas of biology and medicine. The eicosanoids comprise the prostaglandins, thromboxanes and leukotrienes and are products of arachidonic acid, an essential polyunsaturated fatty acid stored in tissue phospholipids. At present we may be at the cross-roads in the therapeutic approaches we have for the treatment of the 100 or more rheumatic conditions. This is be cause we now recognise that although some advances have been made with the development of a large range of non-steroidal and steroidal drugs during the past two decades or so, we now recognise that many, if not all, of these have rather limited effects on many of the disease processes which underlie the manifestations of the various rheumatic states. Advances in molecular bi- 010gy in the past 5-10 years have enabled these tools to be applied extensive ly for developing further our understanding of the rheumatic disease processes. In some cases these molecular tools (e. g. ,),-interferon, interleukin- 2, T-cell antibodies) have been directly employed as therapies themselves. While the outcome from trials with such agents in rheumatoid arthritis in particular has not been as would have been hoped, these results as with cy closporin A and low-dose methotrexate in the therapy of rheumatoid arth ritis have given us important indications for the approach employing what are generally described as "immunomodulators" to control this disease. But this may not be the same type of approach which is desirable for all types of rheumatic conditions. Indeed, even the way which the present range of drugs and other therapies are applied may not be the most effective and safe means of treating different types of arthritic conditions.
