This ground-breaking title presents an interdisciplinary introduction to the subject of Dependability and how it applies in medicine generally and in neurology in particular. Dependability is the term applied in engineering and industry to a service that is safe, reliable and trustworthy. Dependable systems use a variety of methods to deliver correct service in the face of uncertainty resulting from misleading, erroneous information, and system faults. Dependable systems result from the application of systematic methods in design, operation, and management to deliver their services. Dependability in Medicine and Neurology presents the philosophy and ideas behind the specific methods of dependability and discusses the principles in the context of medical care and neurologic treatment especially. Patient case vignettes are used widely to illustrate key points. A first-of-its-kind title and based on the author’s many years of teaching these principles to medical colleagues throughout the United States, Dependability in Medicine and Neurology will inspire readers to develop applications for their specific areas of clinical practice. Intended for physicians (especially neurologists), medical students, nurses, and health administrators, Dependability in Medicine and Neurology is an indispensable reference and important contribution to the literature.

This definitive history of the American Board of Psychiatry and Neurology (ABPN) shows how the Board, by defining core competencies in psychiatry and neurology, established national guidelines and standards for certification during an era of unparalleled technical and therapeutic advances. Understanding how the various specialties and subspecialties evolved, and how the credentialing process was conceived, developed, and adapted to reflect changes in clinical practice, is invaluable to both the candidate approaching the certification exams and the diplomate seeking to maintain certification. In addition, the book's emphasis on the future as well as the past provides a framework for thinking about the contemporary issues that confront the individual clinician and the community as a whole. The American Board of Psychiatry and Neurology: Looking Back and Moving Ahead provides persuasive testimony that those earning certification have achieved an exceptional standard of professionalism, expertise, commitment to lifelong learning, and adherence to ethical standards, as well as assurance that the ABPN will continue to provide this service to the profession -- and the public -- at the highest level.

Medical professionalism faces distinctive challenges in the 21st century. In this chapter, we review the history of professionalism, address specific challenges physicians face today, and provide an overview of efforts to address these issues, including behavioral and virtue ethics approaches. First, we discuss core features professions share and the development of codes of medical ethics that guide the practice of western medicine. Second, we address challenges related to the doctor–patient relationship, continuity of care, cultural competence, conflicts of interest, and the regulation of quality of care through maintenance of certification. We then explore three cultural trajectories that have deeply influenced medical practice: the technologic imperative, physicians’ collective neglect of structural factors impacting medicine, and the rise of commercialism. Finally, we describe efforts to address these challenges, focusing on the Physician Charter developed by the American Board of Internal Medicine and widely endorsed by medical boards and societies internationally.
