This concise practical guide is designed to facilitate the clinical decision-making process in the management of rotational atherectomy procedures by reviewing a number of cases and defining the various diagnostic and management decisions open to clinicians. It will be well illustrated but concise, enabling the reader to obtain relevant information regarding both standard and unusual cases in a rapid, easy to digest format. Each case will also include a narrative description and patient management tips. Rotational atherectomy (‘rotablator’) is used to treat heavily calcified lesions within coronary arteries at the time of percutaneous coronary intervention (PCI). Lesions that are inadequately treated may not be dilatable by balloons; and stents that are implanted in such lesions are at high risk for acute stent thrombosis and stent failure. However, rotablator therapy is high risk and present unique challenges to the interventionalists. Complications are generally uncommon. However, when they do occur, they tend to be disastrous. In this case book, we present a series of rotablator cases: basic, complex cases, difficult cases, complications and unusual situations. Although PCI operators all have some degree of experience with this therapy, each operator is unlikely to have significant experience given the cost of the device and the risk involved. Therefore, this book will provide interventionalists with a valuable compendium of cases which can help them in the management of patients whom they plan to treat with rotational atherectomy.

Each complication is presented first with a case example: case description; how it was dealt with; why it happened; and a discussion of whether and how it could have been avoided in this case. Following is a broader presentation providing more context on why the specific complication may occur; how to anticipate it; how to avoid or minimize it if possible and how to address it if it occurs. Key points in bulleted format are presented as "Lessons" highlighted throughout the text. Complications of Interventional Cardiovascular Procedures is concise and presents information succinctly for efficient learning and quick clinical reference. A companion DVD presents selected video loops of the procedures. This book provides comprehensive coverage of all aspects of interventional cardiology, across five sections, further divided into 88 chapters. The first section covers the evolution periprocedural pharmacology, beginning with chapters on the history of coronary intervention and concluding with clinical cases. The second section covers specific coronary interventions, taking either a disease-based or an anatomical approach. The chapters also provide information on individual patient groups, such as the elderly and diabetics. Detailed chapters on a range of devices used in interventional cardiology are included in this section. Further sections cover a wide range of peripheral and structural interventions, and the final chapter on general topics includes radiation protection, prevention and management of bleeding, and haemodynamic essentials. A unique combination color atlas and synoptic text on interventional cardiology procedures Within the field cardiovascular medicine, interventional cardiology has evolved rapidly in the last decade. In particular, there are advanced therapeutic valvular interventions that compare to historical outcomes with surgical approaches. Transcatheter aortic valve replacement may soon become the standard of care, and transcatheter mitral valve interventions continue to develop rapidly. In addition, improvements in drug-eluting stent technology continue to improve long-term outcomes. Keeping in mind of these rapid advances in interventional cardiology, we have put together this color atlas and synopsis of interventional cardiology to enable medical students, postgraduate physician trainees, practicing clinicians, including physician assistants, nurse practitioners, internal medicine physicians, and general practitioners so that they can rapidly assimilate information on a wide variety of interventional cardiology related topics. This practical, highly clinical resource fills the need in the literature for an interventional cardiology reference that provides expert textual guidance on patient diagnosis and management, along with a comprehensive collection of images (many in full color) illustrating specific procedures. FEATURES: •Logically divided into four sections: The Hemodynamic Study, Percutaneous Coronary Interventions, Non-Coronary Cardiac Interventions, and Peripheral Interventions •Strong focus on normal anatomy, radiographic anatomy, indications, contraindications, interpretations, and patient management •Most chapters begin with a case scenario, putting the issue into clinical, real-world perspective. This beautifully illustrated, concisely written resource is perfect for busy cardiologists of all specialties who require broad coverage of the topic in a convenient, quick-access presentation.
