This is a clinician's and researcher's reference text on using calcium antagonists - calcium channel blockers - in the treatment of gestational hypertension and pre-eclampsia. It contains twelve chapters covering experimental and clinical studies on the major calcium antagonists, their definition and classification and pharmacological profile, which drugs to use and when and which drugs to avoid. It also describes nifedipine pharmacodynamics and pharmacokinetics and covers nifedipine first-line treatment of gestational hypertension and pre-eclampsia along with chapters on second-line treatment and nifedipine blood cells and blood rheology. The book provides a comparison between two calcium antagonists in treating gestational hypertension and new categories to classify pre-eclampsia. Includes bibliographic references and index.

Hypertensive disorders of pregnancy are an important cause of severe morbidity, long-term disability and death among both pregnant women and their babies, and account for approximately 14% of all maternal deaths worldwide. Improving care for women around the time of childbirth is a necessary step towards achievement of the health targets of the Sustainable Development Goals (SDGs). Efforts to prevent and reduce morbidity and mortality during pregnancy and childbirth could also help address the profound inequities in maternal and perinatal health globally. To achieve these goals, healthcare providers, health managers, policy-makers and other stakeholders need up-to-date and evidence-based recommendations to inform clinical policies and practices. In 2019, the Executive Guideline Steering Group (GSG) on WHO maternal and perinatal health recommendations prioritized issuing new WHO recommendations on antihypertensive drugs for non-severe (mild to moderate) hypertension during pregnancy in response to new important evidence on this intervention. For this guideline, non-severe hypertension and mild to moderate hypertension is used interchangeably, defined as diastolic blood pressure of 90–109 mmHg.

This thoroughly revised and expanded new edition presents the current state of knowledge of the diverse applications of calcium antagonists and makes recommendations about their appropriate use. Chapters have been added to critically review the new applications of these agents and to address the concept of fixed-dose formulations--the combination of calcium antagonists with ACE inhibitors or beta blockers.

Hypertensive disorders are among the most common medical complications of pregnancy, with an incidence of approximately 6-10%. This spectrum of conditions includes essential hypertension, pre-eclampsia and HELLP syndrome. For patients with pre-existing hypertension, management ideally commences prior to conception, and continues through pregnancy to the postnatal period. This book provides information on the evidence-based management of women with hypertension throughout pregnancy, supported by important background information on the etiology, risk-factors and pathophysiology of these disorders. Illustrated with accompanying algorithms, tables and lists for quick reference on diagnostic criteria, drugs and side-effects, this book will help clinicians rapidly gain access to the information they need to care for these patients. This will to be of interest to all grades of obstetric trainees as well as specialists, obstetric anesthetists and anesthetic trainees, midwives and maternal-fetal physicians.
