The International Workshop - Conference on Atherosclerosis was held at the University of Western Ontario, London, Ontario, Canada, September 1 - 3, 1975. This book does not represent in a strict sense the entire proceedings of the above Workshop - Conference, but does reflect largely the format and the essential content of the scientific sessions. Thus, each of the three Sections of the book is comprised of the summarized presentations either at the Plenary Sessions (Section I), Proffered Papers (Section II) or Workshops (Section III). Section I comprises all the presentations of the Plenary Session on September 1 and the first three presentations at the Plenary Session on the last day of the Conference (September 3). The remaining two addresses of the latter Session (Resume of Workshop - Conference and Closing Remarks) follow the Section III at the end of the book. Sections II and III are subdivided into Chapters which correspond to the individual Sessions of Proffered Papers and Workshops, respectively. To facilitate the orientation, particularly for those who attended the Workshop - Conference, a Summary Table of all Sessions of Proffered Papers designated as Chapters in this book, precedes Section II, and a similar Summary Table of Workshops, also designated as Chapters, precedes Section III. Seymour Glagov The last meeting, devoted exclusively to an examination of the atherosclerotic plaque, took place in Chicago 25 years ago under the joint auspices of the Council on Arteriosclerosis of the American Heart Association and the Chicago Heart Association. The proceedings were published subsequently in a volume entitled "Evolution of the Atherosclerotic Plaque", edited by Richard J. Jones (1). Both experimental and human lesions were considered and several provocative new approaches to the disorder were discussed. The electron microscope was being applied systematically to the study of blood vessels at that time, so that details of the infrastructure and cellular composition of the artery wall and of atherosclerotic lesions were presented in some detail. There was, as one result of these explorations, considerable discussion of morphologic evidence suggesting that the principal cell involved in the atherogenic process was neither the fibroblast nor the macrophage, as had been supposed, but the smooth muscle cell. In particular, the findings indicated that this cell could incorporate lipid and become a foam cell.

Even though numerous questions with regard to the pathogenesis of athero sclerosis have not yet been answered, the accumulated evidence indicates significant regression of lesions in experimental animals. This is discussed extensively in this monograph, as are the mechanisms involved in regression of lesions. Whether human atherosclerosis has the potential for regression appears to be the most important, but at the same time the most difficult question to answer. Contrary to experimental atherosclerosis in animals, which can be produced and which can regress within a few months, human lesions in general develop slowly over many years. Therefore, measures aimed at modifying this process may also require many years to be successful. In addition, repeated direct examination of lesions in the human is usually not possible. Nevertheless, recent reports in patients with hyperlipoproteinemias indicate that pronounced and maintained control of hyperlipidemias may lead, even within months, to regression as evidenced by angiography or sophisticated measurements of peripheral circulation. The monograph is divided into two sections. The first will deal with of lipid deposition in the arterial wall, whether "atherogenesis": mechanisms or not there is evidence of monoclonal origin of human atherosclerosis plaques, cell culture and factors that stimulate smooth muscle proliferation, and animal models of atherogenesis. This section is concluded with a discussion of dietary factors other than lipids in atherogenesis.
