Schizophrenia is often associated with an inadequate response to pharmacological and non-pharmacological treatments. How to treat patients who have an unsatisfactory response to anti-psychotics, including clozapine - which is unequivocally the most powerful antipsychotic medication for this recalcitrant population - remains a clinical conundrum. A range of adjunctive medications have been tried with mixed results; there has also been renewed interest in the role of neuromodulatory strategies, electroconvulsive therapy, and cognitive and vocational approaches. Perhaps a bright spot for the future lies in the evolution of pharmacogenetic approaches for individualized care. In this book, leading experts from Europe, Australia and the Americas provide a timely appraisal of treatments for the most severely ill schizophrenia patients. This clinically focused book is informed by the latest research on the neurobiology and treatment of schizophrenia. It is comprehensive in scope, covering current treatment options, various add-on approaches, and a range of psychosocial treatments. The contributors are respected experts who have combined their clinical experience with cutting-edge research to provide readers with authoritative information on fundamental aspects of clinical care for schizophrenia.

This unique and timely book examines the basic and clinical sciences relevant to schizophrenia and relates them to current treatment strategies. Through describing current thinking on the aetiology and pathophysiology of schizophrenia as relevant to the psychopharmacology of the disorder, the reader gains a fuller understanding of the rationale for psychopharmacological approaches to treatment. Neuroimaging and neuropsychology are covered in detail as well as further chapters on clinical strategies for treatment of schizophrenia with both conventional and atypical antipsychotic treatments. Practical advice on the management of schizophrenia in the community is also given. Bringing together new developments and advances in the field, while discussing important issues relevant to the long-term treatment necessary for this disorder, the result is an essential book for psychiatrists and psychopharmacologists at all levels.

In line with other volumes in the Neuroscience Perspectives Series, this volume covers the background, pharmacology, molecular biology, and biochemistry of antipsychotic drugs, together with an overview assessment of the therapeutic considerations. Over the past 40 years, the effectiveness of conventional neuroleptic agents for psychotic illness has been offset by a wide range of adverse side-effects, including motor side-effects like parkinsonism. Studies show that lowering doses may still produce the antipsychotic effect while lessening the risk of side-effects. As all available antispychotic drugs are able to block dopamine, specifically D2 receptors, doses below the threshold level for producing acute motor disorder can still be therapeutically effective. With the identification and characterization of multiple dopamine receptors, the possibility of more selective drugs with better side-effect potential has arisen. Other novel antipsychotic agents include D1 receptor blockers, partial dopamine agonists and non-dopamine drugs such as 5-HT receptor blockers, sigma receptor antagonists and NMDA receptor agonists. This volume reviews both the basic science of the conventional and atypical neuroleptics and their present and potential therapeutic use.

Many people consider stopping their psychiatric medications, but prescribers may not know how to do this in a collaborative, systematic way. This book describes the ins and outs of how clinicians can work closely with their patients to consider whether or not to try decreasing medications. It outlines the how and when, and gives recommendations on what the prescriber and patient may encounter along the way.
