This playful and practical handbook presents one of the first clinically tested treatment protocols for Functional Abdominal Pain (FAP) in young children. Created and tested by Dr. Nancy Zucker with support from the National Institutes of Health, this intervention teaches children to become “FBI agents” (Feeling and Body Investigators) – detectives who investigate and learn to manage their symptoms. Kids develop a newfound trust in their bodies by using strategies that transform fear and confusion into curiosity and humor. This clinical manual provides a detailed step-by-step guide to treatment, including session plans, worksheets and activities, and is supplemented by diagrams, excerpts from case dialogues, references and cartoons. Downloadable full color materials are available online to print and use in individual or group sessions. Offering an effective and fun approach firmly rooted in science, this manual guides clinicians in implementing FBI in their own practices, enabling more children to access this novel treatment.

"Chronic abdominal pain represents a common problem in children. In almost 90% of children presenting with chronic abdominal pain, no organic cause is found and a diagnosis of functional abdominal pain is made. Initially this condition was referred to as 'recurrent abdominal pain' by Apley and Naish and it is currently defined as abdominal pain-related functional gastrointestinal disorders (AP-FGIDs); divided into functional dyspepsia (FD), irritable bowel syndrome (IBS), abdominal migraine (AM), functional abdominal pain (FAP) and functional abdominal pain syndrome (FAPS) according to the Rome III criteria. Children with AP-FGIDs report impaired health-related quality of life in relation to physical complaints and AP-FGIDs are ranked as second in causing school absence. The underlying pathophysiology of AP-FGIDs is considered to be multifactorial, but needs further elucidation. This lack of understanding often leads to extensive investigations, non-effective therapeutic modalities and staggering healthcare costs. This thesis elucidates current and new knowledge regarding epidemiology, pathophysiology, diagnostic workup and treatment of functional abdominal pain to improve the understanding and to maximize the quality of care for children suffering from this condition. The focus of this thesis is to identify risk factors for developing pediatric AP-FGIDs and to evaluate diagnostic and therapeutic strategies. This thesis ends with the evaluation of a new complementary treatment option."--Samenvatting auteur.

"Functional abdominal pain disorders (FAPDs) are common chronic conditions among children and adolescents. These disorders are characterized by long lasting abdominal pain (longer than 2 months), where the gastrointestinal symptoms cannot be fully explained by another medical condition after appropriate clinical evaluation. According to the Rome IV criteria, four FAPDs subtypes exist: (1) functional dyspepsia (FD), (2) irritable bowel syndrome (IBS), (3) abdominal migraine (AM), and (4) functional abdominal pain – not otherwise specified (FAP-NOS). Pediatric FAPDs are associated with impaired quality of life, increased school and/or work absenteeism, and a higher risk on anxiety or depressive symptoms compared to healthy individuals. It is thought that FAPDs derive from a complex interplay of biopsychosocial factors, causing dysregulation of the gut-brain axis via immunological, hormonal and neural pathways, possibly leading to visceral hypersensitivity and gastrointestinal dysmotility. Since the exact pathophysiology of pediatric FAPDs is still not completely understood and the available treatment options are diverse, adequate management is sometimes challenging. In this thesis, we focused on the clinical evaluation of patients in order to customize FAPDs management. We evaluated the use of pictograms to improve symptom evaluation and assessed the impact of the presence of nausea (a common comorbid symptom in children with FAPDs) on both clinical and psychological characteristics of these patients. Moreover, we described the different management strategies of FAPDs. Current strategies in pediatric FAPDs management include education and reassurance, dietary interventions, non-pharmacological (psychosocial) and pharmacological treatment regimens. For children who continue to experience symptoms despite the available interventions, new treatment strategies might be beneficial. Therefore, we also discussed innovative future therapeutic approaches in this thesis."--

Advances over the past two decades have enabled physicians to revolutionize the manner in which they can assess and manage children’s pain. Thirty years ago it was thought that young children did not experience pain and therefore it was not necessary to treat it. Today professionals from a variety of disciplines have contributed data that have revolutionized medical perspectives. Technological advances now enable doctors to treat acute pain in fetuses, premature neonates, infants, toddlers, children, and adolescents with increasing precision and efficacy. Research highlighting the context of chronic pain has moved them away from a mind-body dichotomy and toward an integrated, holistic perspective that leads to substantial improvement in children’s adaptive functioning as well as subjective discomfort. This book covers these topics and is intended for anyone who provides medical care to children. Each chapter provides an overview of the problem, followed by a "hands on" description of relevant assessment and intervention strategies. The role of the primary care practitioner is highlighted, both as a front-line resource as well as a consumer of specialized pediatric pain treatment services. Each chapter ends with a summary and specific bullet points highlighting the most central elements, making for quick and easy reference.

This handbook offers a comprehensive review of cognitive behavioral therapy (CBT) for working in integrated pediatric behavioral health care settings. It provides research findings, explanations of theoretical concepts and principles, and descriptions of therapeutic procedures as well as case studies from across broad conceptual areas. Chapters discuss the value of integrated care, diversity issues, ethical considerations, and the necessary adaptations. In addition, chapters address specific types of pediatric conditions and patients, such as the implementation of CBT with patients with gastrointestinal complaints, enuresis, encopresis, cancer, headaches, epilepsy, sleep problems, diabetes, and asthma. The handbook concludes with important directions in research and practice, including training and financial considerations.Topics featured in this handbook include: Emotional regulation and pediatric behavioral health problems. Dialectical Behavior Therapy (DBT) for pediatric medical conditions. Pharmacological interventions and the combined use of CBT and medication. CBT in pediatric patients with chronic pain. CBT for pediatric obesity. CBT-informed treatments and approaches for transgender and gender expansive youth. Medical non-compliance and non-adherence associated with CBT. Training issues in pediatric psychology. The Handbook of Cognitive Behavioral Therapy for Pediatric Medical Conditions is an essential resource for researchers and graduate students as well as clinicians, related therapists, and professionals in clinical child and school psychology, pediatrics, social work, developmental psychology, behavioral therapy/rehabilitation, child and adolescent psychiatry, nursing, and special education.

Children and adolescents frequently report chronic pain, with many experiencing recurring or persistent abdominal pain occurring in the absence of organic disease, often referred to as functional abdominal pain (FAP), and/or chronic abdominal pain (CAP). Research suggests that CAP and/or FAP is the result of a complex interplay of physiological, psychological, and social processes and is associated with significant disruptions in daily functioning, co-morbidity with anxiety and/or depression, and complicated family dynamics which serve to support pain behaviors. Children with CAP/FAP present as challenging cases for primary care physicians (PCPs) to manage. The present dissertation includes a literature review of CAP and FAP. Although the literature review suggests that CAP and FAP are complex issues that many children and families experience, there is a body of scholarly literature that includes important treatment recommendations and considerations. However, through general internet searches (using Google) and a review of scholarly literature (using Psych Info), it is evident that there is a lack of readily available resources for the families of children with FAP and CAP. This dissertation project includes a user-friendly, psycho-educational resource for parents and an accompanying pamphlet for PCPs to help introduce the guidebook to families, both of which are grounded in scholarly literature and the available research base.
