Metabolic syndrome (MetS), including obesity, hyperglycemia, hyperlipidemia, hyperinsulinemia, are associated with disturbed energy homeostasis, limited whole-body insulin sensitivity, and an increased risk of cardiovascular and chronic kidney disease (CKD). The prospective and cross-sectional studies have shown a close association between the MetS and CKD. Chronic inflammation and vascular dysfunction are two key MetS-related etiological factors for renal injury. CKD is characterized by mitochondrial dysfunction, oxidative stress, and aberrant autophagy at the cellular level, which is also typical of metabolic disorders. However, the relationship and mechanism between metabolic disease and chronic kidney dysfunction still call for deeper understanding. This Research Topic aims to gather both Reviews and Original Research articles focused on discovering the truth behind the phenomenon that glucose and lipid metabolic disorders and obesity have important public health implications in terms of individuals with CKD. The studies covering new insights to determine the link between MetS-related immunity response and renal dysfunction will also be considered.

That precursors of adult coronary artery disease, hypertension, and type II diabetes begin in childhood have been clearly established by the Bogalusa Heart Study. This unique research program has been able to follow a biracial (black/white) population over 35 years from childhood through mid-adulthood to provide perspectives on the natural history of adult heart diseases. Not only do these observations describe trajectories of cardio-metabolic risk variables leading to these diseases but provide a rationale for the need to begin prevention beginning in childhood. The trajectories of the burden of cardio-metabolic risk variables in the context of their fetal origin and chromosome telomere dynamics provide some insight into the metabolic imprinting in utero and aging process. The observed racial contrasts on cardio-metabolic risk variables implicate various biologic pathways interacting with environment contributing to the high morbidity and mortality from related diseases in our population. To address the seriousness of the onset of cardiovascular disease in youth, approaches to primordial prevention are described focussing on childhood health education as an important aspect of Preventive Cardiology.

During the past decade, experimental and clinical studies have suggested that dyslipidemia may be an important risk factor for the progression of renal disease. This volume explores in great detail recent advances in our understanding of the pathogenesis and treatment of this common complication of progressive renal injury. In the experimental investigations presented, emphasis is placed on specific disturbances of lipids that are seen in progressive renal disease including the effects of oxidatively modified lipoproteins and Lp(a) on various functions of the glomerular cells. Clinical studies have identified dyslipidemia as a risk factor for progressive renal disease in diabetes mellitus, various non-diabetic renal diseases, and most recently in patients with progressive renal allograft loss. An emerging interest in the genetics of dyslipidemia has recently arisen and this topic as it specifically relates to progressive renal disease is discussed. The final section of the book offers new insights into the mechanisms of action of antilipemic therapy used in the treatment of dyslipidemia. This volume should be read by all nephrologists caring for patients with progressive renal disease and by physicians interested in the biology of lipids, diabetes and essential hypertension.

Diabetes and hypertension have evolved as two of the modern day epidemics affecting millions of people around the world. These two common co-morbidities lead to substantial increase in cardiovascular disease, the major cause of morbidity and mortality of adults around the world. In Diabetes and Hypertension: Evaluation and Management, a panel of renowned experts address a range of critical topics -- from basic concepts in evaluation and management of diabetes and hypertension, such as dietary interventions, to evaluation and management of secondary hypertension in clinical practice. Other chapters focus on high cardiovascular risk populations such as those with coronary heart disease, chronic kidney disease and minority patients. In addition, evolving concepts and new developments in the field are presented in other chapters, such as prevention of type 2 diabetes and the epidemic of sleep apnea and its implication for diabetes and hypertension evaluation and management. An important title covering two of the most troubling disorders of our time, Diabetes and Hypertension: Evaluation and Management will provide the busy practitioner with cutting edge knowledge in the field as well as practical information that can translate into better care provided to the high-risk population of diabetics and hypertensive patients.

The present book contains the Proceedings of a two day Symposium on Uremic Toxins organized at the University of Ghent in Belgium. A series of guest lectures, free communications and posters have been presented. An international audience of 163 scientists from 16 nationalities listened to and discussed extensively a spectrum of topics brought forward by colleagues and researchers who worked for many years in the field of Uremic Toxins. There is a striking contrast between all the new dialysis strategies available in the work to "clean" the uremic patients and the almost non-progression of our knowledge on uremic toxins in the past decade. In this sense the symposium was felt by all participants as a new start for the research in the biochemical field of the definition of uremia. If the present volume would stimulate new work in this field in order to define uremia, or identify the uremic toxins, the purpose of the organizers would be maximally fulfilled.
