In 1984, a year after Congress overhauled the way Medicare would pay hospitals and medical centers, a young entrepreneur named Richard Scrushy decided to start a new kind of healthcare company - one that took rehabilitation out of the basements of hospitals and moved it into bright and lively settings that promoted wellness and healing. Scrushy sought to coordinate all aspects of rehabilitation, from social services to physical and occupational therapy, under one roof. After only 18 years, HealthSouth became the nation's leading provider of ambulatory surgery, diagnostic imaging and inpatient and outpatient rehabilitation. Brings to vivid life the fascinating history of one of the most dynamic healthcare corporations in America.

This book investigates the connection between tightening mobilization constraints and the use of PMSCs in the United States, the United Kingdom, and Italy. Drawing on neoclassical realism and institutionalist theory, it conceptualizes democracies’ use of private military and security companies (PMSCs) as an attempt to circumvent the tightening constraints on the mobilization of military power. The use of private military contractors is less subjected to parliamentary restrictions and less visible to public opinion than the deployment of soldiers. Rather than cheaper in financial terms, PMSCs are therefore politically cost-effective, as they enable decision-makers to minimize the institutional obstacles on conducting military operations and the electoral costs attached thereto. The need to reduce the ex ante hurdles and the ex post costs of military deployments fills the blind spots of alternative explanations for the use of PMSCs based on effectiveness, ideology, and organizational interests.

With sales of more than $13 billion, IBP is a world-leading producer of protein. In fact, one out of every three steaks and one of every five pork chops consumed in America today was produced by IBP. Yet remarkably, the company is less than 30 years old. Founders Andy Anderson and Currier J. Holman built IBP by applying seemingly simple concepts to one of America's most established industries. Instead of shipping live cattle across the country to stockyards in Chicago and on the East Coast, Currier Holman located processing plants in Iowa and Nebraska near the largest ranches. From there, they shipped vacuum - packed boxed beef, already trimmed and packaged, to supermarkets across the country. With this single idea, and an unwavering attention to quality and efficiency, IBP transformed the industry.
