Why write another book on ethics? As practitioners we are involved both in the design and delivery of services to people with mental health problems. In common with all other professionals, our work has led to the experience of ethical dilemmas: typically, these have involved major confrontations, either with our col leagues or our consciences. This book, however, is not limited to a discussion of such major themes. Rather, we have tried to use a broader canvas: ethics, in our view, is really about the judgement of right and wrong in ordinary, everyday life. Ethics are highly personal: we fashion our own personal code from our experi ence of others, and from the 'tests' which bring meaning to our lives. Such experiences shape our individual values. We bring these codes and values to our work. We are not always aware of their influence in our dealings with people. Although we may not always be aware of it, all our actions pose an ethical question. Given that our work involves us in helping others to live ordinary, satisfying lives, this challenge heightens the intensity of our ethical dilemmas. This is most evident where our personal code conflicts with the implicit code of the health setting.

Objective:To identify the ethical challenges faced by psychiatric nurses in clinical environments through focus group interviews. Backgrounds:With the various known ethical challenges related to psychiatry, the usefulness of conferences focusing on such challenges has been reported in Japan. On the other hand, care actually provided in psychiatric hospitals remains unethical. In such a situation, identification of the ethical challenges faced by psychiatric nurses may have a marked significance. Methods:This study adopted a qualitative, descriptive design. A focus group interview was conducted with nurses with extensive experience in psychiatric nursing. The nurses freely talked about psychiatric nursing and ethics. The interview data were subsequently organized as narrative records. The study was approved by the ethics committee of the facility the researchers belonged to. Results:14 nurses were divided into two groups. The following ethical challenges were identified: 1) insufficient physical and end-stage care for persons with mental disorders, 2) punitive treatment for inpatients, 3) continuous deprivation of patient liberty, 4) care based on the instructions of psychiatrists who administer placebos, 5) overbearing attitudes toward patients, 6) excessive restrictions on patients to avoid accidents (prevent suicide), 7) insufficient approaches to reduce patientsu2019 anxiety during behavioral restrictions, and 8) not telling patients the truth. Additionally, the nurses described their mental conflicts due to difficulty in sufficiently discussing these challenges among multiple professionals. Conclusion:The nurses regarded the current situation as inevitable with a sense of resignation, while questioning some measures, such as placing severer restrictions on patients to prevent suicide, leaving hospital, or other serious events. Psychiatric nurses, who frequently face ethical challenges related to behavioral restrictions and involuntary inpatient treatment, need to continuously examine appropriate ethical nursing practice, and address these challenges. As a future perspective for the realization of multi-professional collaboration conferences, it may be important to clarify the roles of psychiatric nurses.
