Like so many Americans, I am horrified and outraged at the continued devastation that we see in the Gulf Coast. There's been a lot of talk about the economic and the environmental impact, and I want to make sure we don't forget about the oil and gas industry workers, who deserve to be protected: the 11 workers who were killed at the Deepwater Horizon; the 15 workers who died, and more than 170 injured, at the BP Texas City refinery disaster in 2005; the 7 workers who were killed at the tragic fire at the Tesoro refinery in my home State, in Anacortes, WA, earlier this year; and the hundreds more who've been injured or killed at refineries, on rigs, and in other oil and gas facilities over the past several years. Let's be clear. Despite what anyone tries to say, this is not a safe industry.

To be able to compete successfully both at national and international levels, production systems and equipment must perform at levels not even thinkable a decade ago. Requirements for increased product quality, reduced throughput time and enhanced operating effectiveness within a rapidly changing customer demand environment continue to demand a high maintenance performance. In some cases, maintenance is required to increase operational effectiveness and revenues and customer satisfaction while reducing capital, operating and support costs. This may be the largest challenge facing production enterprises these days. For this, maintenance strategy is required to be aligned with the production logistics and also to keep updated with the current best practices. Maintenance has become a multidisciplinary activity and one may come across situations in which maintenance is the responsibility of people whose training is not engineering. This handbook aims to assist at different levels of understanding whether the manager is an engineer, a production manager, an experienced maintenance practitioner or a beginner. Topics selected to be included in this handbook cover a wide range of issues in the area of maintenance management and engineering to cater for all those interested in maintenance whether practitioners or researchers. This handbook is divided into 6 parts and contains 26 chapters covering a wide range of topics related to maintenance management and engineering.

In petroleum refineries, the concern for safety could be said to focus on two main areas: process safety and labor or personnel safety. These two areas are very different from each other, but both are important. They require two very different approaches to what one might call the safety problem. Process safety involves the development and implementation of interventions concentrated on preventing or minimizing the effects of loss of containment of flammable, toxic, or reactive chemicals. Labor or personnel safety interventions focus on the prevention or mitigation of hazards that can result in individual injuries or exposures; a much less catastrophic problem, but a no less important one. Through both approaches to the safety problem, we attempt to prevent or minimize the impact of accidents. There are many definitions for an accident, but for the purposes of this chapter, they will be considered unintentional events that result in undesirable consequences. Accidents occur in nearly every facet of our lives. Because in petroleum refineries, there is extensive heavy manual work (especially during maintenance turnarounds); there is large, heavy equipment; there are toxic, flammable and reactive chemicals; and there are high temperatures and pressures in many processes, it is no surprise that accidents occur here. Over the last 2030 years, in petroleum refineries and similar processes, there have been several significant watershed incidents that have fueled the development of many of the regulations and prevention activities that apply today and that serve to minimize the impact of the hazards typically associated with petroleum refining. The driving forces behind much of what makes up a modern safety and health program in the petroleum refinery in the United States are the regulatory agencies: the Occupational Safety and Health Administration (OSHA) and the U.S. Environmental Protection Agency (EPA). Although this chapter will draw heavily from these regulations, it will also include discussion on the ways and means that companies comply with these regulations (as well as other consensus standards and general good practice). The content of typical safety and health programs in petroleum refineries will also be covered, but the primary focus of this chapter will be on process safety, safe work practices or permit-to-work systems, and behavioral safety.

First-line managers have to maintain the integrity of facilities, control manufacturing processes, and handle unusual or emergency situations, as well as respond to the pressures of production demand. On a daily basis, they are closest to the operating personnel who may be injured by a process accident, and they are in the best position to spot problem conditions and to act to contain them. This book offers these managers "how-to" information on process safety management program execution in the operations and maintenance departments, recommending technical and administrative process safety activities for the entire life cycle of the plant. Helpful tables and references add to the value of this process safety resource.

Process safety is a blend of engineering and management systems and covers how major hazards arising from process industries are identified, assessed and controlled. This book introduces the basics of process safety from concept selection, design through operation and maintenance in the petroleum industry. It is invaluable for undergraduate students of safety engineering and loss prevention engineers in the hydrocarbon industry.

Shutdown management is project management of a special kind: managing the repair, replacement or maintenance of critical systems. Manufacturing and process plants, computer systems, airliners, and many other systems must be regularly closed down or taken out of service for planned maintenance operations. This book provides a complete shutdown project planning guide along with a new, detailed model of excellence and step-by-step project guide. In a critical field, this book shows the maintenance manager or project leader how to get the job done correctly.*Covers all aspects of major maintenance project planning, minimizing downtime and improving maintenance schedules *Covers projects ranging from weekend overhauls through to complete plant rebuilds*With detailed checklists and a new step-by-step project guide
