Over the course of the last two decades, improved practices in child and adolescent mental healthcare have led to a decreased environment of stigma, which also led to an increased identification and treatment of mental health disorders in children and youth. Considering that treatment and outcomes are improved with early intervention, this is good news. However, the success gained in the field of child and adolescent psychiatry leads to a new challenge: transitioning from adolescent care to adult care. It has been known for some time that children, adult, and geriatric patients all have unique needs where it comes to mental healthcare, yet limited work has been done where it comes to the shifting of the lifespan. Where it comes to the child-adult transition—defined as those in their late teens and early/mid-20s—there can be multiple barriers in seeking mental healthcare that stem from age-appropriate developmental approaches as well as include systems of care needs. Apart from increasing childhood intervention, the problem is exacerbated by the changing social dynamics: more youths are attending college rather than diving straight into the workforce, but for various reasons these youths can be more dependent on their parents more than previous generations. Technology has improved the daily lives of many, but it has also created a new layer of complications in the mental health world. The quality and amount of access to care between those with a certain level of privilege and those who do not have this privilege is sharp, creating more complicating factors for people in this age range. Such societal change has unfolded so rapidly that training programs have not had an opportunity to catch up, which has created a crisis for care. Efforts to modernize the approach to this unique age group are still young, and so no resource exists for any clinicians at any phase in their career. This book aims to serve as the first concise guide to fill this gap in the literature. The book will be edited by two leading figures in transition age youth, both of whom are at institutions that have been at the forefront of this clinical work and research. This proposed mid-sized guide is therefore intended to be a collaborative effort, written primarily by child and adolescent psychiatrists, and also with adult psychiatrists. The aim is to discuss the developmental presentation of many common mental health diagnoses and topics in chapters, with each chapter containing clinically-relevant “bullet points” and/or salient features that receiving providers, who are generally, adult-trained, should keep in mind when continuing mental health treatment from the child and adolescent system. Chapters will cover a wide range of challenges that are unique to transition-age youths, including their unique developmental needs, anxiety, mood, and personality disorders at the interface of this development, trauma and adjustment disorders, special populations, and a wide range of other topics. Each chapter will begin with a clinical pearl about each topic before delving into the specifics.

That next decade has happened, and Marmor's papers remain a beacon of professional endeavor. This second edition includes a final chapter on "Short-Term Dynamic Psychotherapy," in which the historical background of brief psychotherapy, focusing on the contributions of Freud, Ferenczi, Rand, and Alexander, is examined and synthesized. Throughout, certain basic themes stand out. First is the necessity for building upon a solid foundation of scientific thought, coupled with a readiness to change theories that do not fit with established facts. Second, Marmor offers a systems theory to replace simplistic, unitary, or linear theories. Third, he presents some common denominators for illuminating the divergent views that characterize contemporary psychiatric theory and practice. The whole is linked by a deep concern with betterment of the human predicament. Marmor demonstrates that causation in psychiatry can be optimally understood in terms of multiple interacting variables rather than as a response to unitary factors. He foreshadows and predicts developments that are now current in contemporary psychiatric practice, such as the relationship between neurochemistry and behavior, and group therapy with dynamic psychotherapy. He also deals with the importance of cultural and socioeconomic factors in individual personality development. The work concludes with a series of chapters on interethnic hostility, nationalism, and urban violence. Marmor's work clarifies the nature of the psychoanalytic process by liberating it from obscurantism and jargon. This book points the way toward unraveling some of the cognitive dissonance in this area. As Leon Eisenberg observed, this is "an admirable vade mecum of dynamic psychiatry both for residents in training and clinicians in practice."

Every year the few hundred members of the Committees, Task Forces, and Councils of the American Psychiatric Association meet in Washington, D.C. to conduct their business. They deliberate on a wide variety of issues encompassed in the activities of each group. The psychiatrists constituting this mixed and somewhat elite group include some of the better-known and promising people in the profession, which makes the plenary session and cocktail party good occasions to meet old friends and to make new ones. Several years ago one of us (N.B.L.) attended this gathering as a member of a soon-to-be defunct group, the Committee Liaison with the American College of Physicians, and met Ann Chappell, a member of the Task Force on Women. We were soon joined by Richard Grant. Ann impressed us with the work her group was doing on issues surrounding the Women's Movement as it relates to patients and the changing roles of the early 1970s. She was struck by the fact that although some women had been very active in this endeavor, nobody in Ameri can psychiatry was addressing issues which are arising in men as a result of the changing roles of women in society. Dick and Norm were moved by what she said and decided that they would make an effort to gather together people interested in the issue of the changing roles of males in society at the oncoming meeting of the American Psychiatric Association.
