The purpose of the investigation is to make a determination on whether or not the dam constitutes a hazard to human life or property. Mine No. 60-Pond 5 is located at the headwaters of an unnamed tributary to the Center Branch of Pigeon Creek, in Somerset Township, Washington, County, Pennsylvania. The Mine No. 60-Pond 5 is classified as a 'high' hazard dam. The No. Mine 60-Pond 5 is owned by the Bethlehem Mines Corporation Eight-Four, Pennsylvania. The field inspection of Mine No. 60-Pond 5 was performed on 18 March 1981 and 26 March 1981 and consisted of: Visual observations of the embankment crest and slopes, groins and abutments; Visual observations of the principal (and emergency) spillway including approach channel, overflow crest, and discharge channel; Visual observations of the embankment's downstream toe area including drainage channels and surficial conditions; Visual observations of downstream conditions and evaluation of the downstream hazard potential; Visual observations of the reservoir shoreline and watershed; Visual observations of the outlet works; Transit stadia surveys of relative elevations along the embankment crest centerline, spillway, and across the embankment slopes. The visual inspection, operational history and hydrologic/hydraulic analysis indicate that the facility is in good condition. The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) of the facility ranges between the 1/2-PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and possible loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store approximately 51% of the PMF prior to embankment overtopping at the low top of dam. Thus, the spillway system is considered to be inadequate, but not seriously inadequate. Should the embankment crest be regraded to design elevation, the spillway would be capable of passing approximately 60% of the PMF prior to embankment overtopping.

Based on a review of available information, visual observations of conditions as they existed on the date of the field inspection, and supporting engineering calculations, the general condition of the Montour No. 4 Refuse Bank is considered to be fair. This assessment is based primarily on visual observations of the embankment. The structure is classified as a large size, significant hazard dam. Corps of Engineers guidelines recommend the Probable Maximum Flood (PMF) as the Spillway Design Flood for a large size significant hazard dam. Montour No. 4 Refuse Bank's Spillway Design Flood is the Probable Maximum Flood. Reservoir capacity is 'adequate' because the non-overtopping flood inflow was found, by using the HEC-1 computer program, to be in excess of 100 percent of the PMF.

The dam appears to be in poor condition. No maintenance program exists for the dam and the lack of maintenance has led to a general deterioration of the structure. An upstream shut-off exists for the 12 in. drainline, but the capability of the valve to operate is questionable. Wet areas were observed on the downstream slope of the dam. One area observed at the right abutment contact; and a second wet area was observed on the downstream slope adjacent to the right spillway wingwall, approximately 10 feet below the right crest. The seepage should be investigated to ascertain its effects on the stability of the structure. The spillway is in a deteriorating condition and the concrete in the channel walls and channel bottom should be repaired. Undercutting of the channel floor if left unchecked could lead to the potential failure of the spillway structure. Brush and small trees were observed on the upstream and downstream slope.
