This comprehensive text provides a much-needed review of a disease that is currently garnering the interest of molecular biologists, translational scientists, and clinicians. The volume includes emerging developments in the molecular genetics of endometrial carcinoma. In addition to covering the basic genetics of endometrial carcinoma, chapters also cover a wide range of signaling pathways implicated in endometrial carcinoma. A section of the book includes a number of genetically engineered mouse models, which contribute to understanding the role of various genetic alterations in the development and progression of endometrial carcinoma. These models also provide preclinical models for developing effective targeted therapeutic approaches. Endometrial carcinoma is the most common malignancy of the female genital tract in the United States and the number of cases continues to increase around the world. This book is a meant to serve as a resource for a wide range of scientists, from molecular geneticists to signal transduction biologists, as well as to both clinicians and scientists interested in developing targeted therapeutic approaches for women with endometrial carcinoma.

The endometrium has been the subject of intense research in a variety of clinical settings, because of its importance in the reproductive process and its role in women’s health. In the past 15 years, significant efforts have been invested in defining the molecular phenotype of the receptive phase endometrium as well as of various endometrial pathologies. Although this has generated a wealth of information on the molecular landscape of human endometrium, there is a need to complement this information in light of the novel methodologies and innovative technical approaches. The focus of this International Journal of Molecular Sciences Special Issue is on molecular and cellular mechanisms of endometrium and endometrium-related disorders. The progress made in the molecular actions of steroids, in the metabolism of steroids and intracrinology, in endometrial intracellular pathways, in stem cells biology, as well as in the molecular alterations underlying endometrium-related pathologies has been the focus of the reviews and papers included.]

Abstract: Abstract of thesis entitled Molecular Analysis of Mitochondrial DNA Alterations in Endometrial Carcinomas Submitted by Wang Yue for the degree of Doctor of Philosophy at The University of Hong Kong in June 2005 Recently, mitochondrial DNA (mtDNA) mutations and genetic instability were frequently detected in a number of human cancers. However, the occurrence of mtDNA alterations in endometrial carcinomas is still unknown. This study aims to identify the frequency and characteristics of mtDNA alterations in endometrial carcinomas. To identify somatic mtDNA mutations, DNA sequencing was performed on the mutational hotspot region of mtDNA in endometrial carcinomas and matched normal controls. The frequency of the T→C polymorphism at nucleotide position (np) 16189 was also determined in endometrial carcinomas and compared with the frequencies detected in cervical, ovarian and breast cancers as well as in the general population. The mitochondrial microsatellite instability (mtMSI) in the above four types of cancers was also evaluated. The nuclear MSI (nMSI), the germline mutations of the core genes of the mismatch repair system and methylation status of hMLH1, APC INK4A and p16, were also studied in endometrial carcinomas. 1The copy number of mtDNA in endometrial cancer cells procured by laser capture microdissection (LCM) was determined by real-time quantitative PCR and compared with normal endometrial cells obtained from the same patient. Moreover, by using LCM, mtDNA sequence alterations were analyzed on different loci from the same lesion of a subset of endometrial carcinomas. Finally, the mtDNA copy number and the expression level of mtDNA-encoded genes on estrogen-sensitive RL95-2 cells were quantified by real time PCR, RT-PCR and Western blotting assays. A remarkably high frequency (56%, 28/50) of somatic mtDNA mutations in endometrial carcinomas was observed. MtMSI constituted the majority (89%, 25/28) of the somatic changes detected. And the frequency of mtMSI was significantly higher than the other three types of cancers including cervical, ovarian and breast cancers (χ, P

Objective: To test the Proactive Molecular Risk Classifier for Endometrial Cancer (ProMisE) and determine the frequency of specific/prognostic molecular alterations within a cohort of endometrial cancer (EC) women conservatively treated by combined hysteroscopic resection and progestin therapy.Study design: We used blocks of formalin-fixed paraffin-embedded tissue from the primary tumors of patients enrolled into the ECCo trial (EudraCT 2010-018581-23) between 2007 and 2016. In order to assign EC resectoscopic specimens to one of four ProMisE subgroups, testing involved sequential assessment of i) immunohistochemistry (IHC) for mismatch repair (MMR) proteins MLH1, MSH2, MSH6 and PMS2; ii) sequencing for POLE/POLD1 exonuclease domain mutations (EDMs); iii) p53 IHC.Results: Molecular analysis methods were used in 25 patients (stage IA, G1-2 endometrioid EC), of whom 15 (60%) represented fully evaluable cases. Seven cases (46.7%) had abnormal MMR IHC, POLE/POLD1 EDMs were found in 3 cases (20%), and abnormal p53 IHC in 1 case (6.6%). Three patients (20%) had more than one molecular feature. Among 10 (40%) u2018unclassifiableu2019 patients, six failures in achieving complete molecular categorization were due to the low tumor volume. Molecular classification of the 15 fully evaluable cases yielded the following ProMisE subtypes: 7 (46.7%) MMR IHC abnormal, 1 (6.6%) POLE EDM, 0 (0%) p53 IHC abnormal, 7 (46.7%) p53 IHC wild-type.Conclusions: Although larger series are needed to further assess the feasibility of a molecular categorization in a fertility-sparing setting, data presented are promising. In women with early stage lowvolume disease, operative hysteroscopy could be advantageous to provide samples allowing complete genetic risk assessment.

Endometriosis is defined as the presence of endometrial-like endometrial cells, glands, and stroma outside the uterus, causing a strong inflammatory-like microenvironment in the affected tissue. The exact prevalence of endometriosis is unknown, but estimates range from 2%-10% of women of reproductive age to 50% of infertile women. The etiopathogenesis of endometriosis remains controversial--immune, hormonal, genetic, and epigenetic factors may all be involved, and several theories have been proposed to explain it.This book aims to summarize some of the most recent advancements in basic and translational science (immunology, cell biology, genetics, and epigenetics) in the field of endometriosis that may create new scenarios and change our perspective of the topic.
