From the introduction: "This report is the result of an extensive investigation by the UMWA Department of Occupational Health and Safety concerning the causes of these miners' deaths and related matters. The principal conclusion of this report is that the miners' deaths were a direct consequence of failures by the mine operators and MSHA in designing a mining system that would meet the requirements or intent of the Federal Mine Safety and Health Act."
