Alcohol, tobacco, illegal drugs and psychoactive medication (mis)use are associated with a higher likelihood of developing several diseases, (traffic) injuries and crimes. These substances reduce quality of life and increase the health care and law enforcement costs, productivity losses, etc. Consequently, the social and economic impact of substances on society is substantial. The SOCOST study estimates for the first time social costs for alcohol, tobacco, illegal drugs and psychoactive medication in Belgium for the year 2012. This cost-of-illness study presents the direct costs, the indirect cost as well as the intangible costs related to substance (mis)use. This research was commissioned by the Belgian Federal Science Policy Office (BELSPO) in the framework of the Federal Research Programme Drugs. Two universities cooperated: Ghent University, Institute for International Research on Criminal Policy (IRCP) and the Vrije Universiteit Brussel, Interuniversity Centre for Health Economics Research (I-CHER). The research was conducted under supervision of prof. dr. Freya Vander Laenen, prof. dr. Koen Putman, prof. dr. Lieven Pauwels, prof. dr. Wim Hardyns and prof. dr. Lieven Annemans.

The prevalence rate ranges from 20% - 40% in industrialised countries, and is dramatically increasing in the developing world. Smoking is risky and there is ample scientific evidence to support this statement. We know that smoking is a major cause of disease and premature death, in view of the fact that 3 million people die each year worldwide as a result of their smoking habit. Twenty years ago, the U. S. Surgeon General identified smoking as the single most important cause of morbidity and premature death (USDHEW, 1979). Tobacco consnmption reduces life expectancy vastly. Epidemiological research shows that people who have died from a smoking-related disease would, on average, have lived for an additional 15 years had they not been smokers (Warner, 1987). The economic analysis of tobacco consumption is a complex and challenging issue, which entails addressing many different questions. What is the economic burden of smoking and do smokers "pay their way"? How do individuals perceive their own health risks? What is the effect of the addicting properties of nicotine on the behaviour of a rational, utility maximizing individual? Lastly, what is the most effective way to discourage tobacco consumption? In this context, the assessment of the social burden of smoking using a cost-of-illness framework has played a central role since the beginning of the seventies.

Research on alcohol-related consequences has traditionally focused mainly on health aspects of alcohol consumption or effects which can be more easily quantified or measured. It is evident that alcohol has many consequences which can be characterised as `social' in nature and which are not, or not only, medical and are directly health-related. This book attempts to provide a comprehensive overview of social consequences of alcohol consumption on the individual, group, organisational, and societal level. It is a result of a two-year collaborative study under the leadership of WHO-Euro with the participation of alcohol researchers from Finland, Germany, Norway, Scotland, and Switzerland. Although the book was written by experts in the field, it is targeted not only at scientists, but at all people dealing with alcohol-related problems in practice.
