This report is published quarterly by the Idaho National Laboratory (INL) Performance Assurance Organization. The Department of Energy Occurrence Reporting and Processing System (ORPS), as prescribed in DOE Order 232.2 "Occurrence Reporting and Processing of Operations Information," requires a quarterly analysis of events, both reportable and not reportable, for the previous 12 months. This report is the analysis of 76 occurrence reports and over 16 other deficiency reports (including not reportable events) identified at the INL during the period of October 2013 through December 2013.
