Intensive care medicine is one of the fastest growing services provided by hospitals and perhaps one of the most expensive. Yet in response to the global financial crisis of the last few years, healthcare funding is slowing or decreasing throughout the world. How we manage health care resources in the intensive care unit (ICU) now and in a future that promises only greater cost constraints is the subject of this book, the third in an informal series of volumes providing a global perspective on difficult issues arising in the ICU. Taking 12 developed countries as their focus, leading experts provide a country-by-country analysis of current ICU resource allocation. A second group of experts use the chapters as a departure point to analyze current ICU resource allocation at the level of the global medical village. The process is repeated, but with an eye toward the future – first country by country, then at the global level – that takes into account initiatives and reforms now underway. A fictional healthcare plan, the “Fair & Equitable Healthcare Plan,” is put forth to address weaknesses in existing approaches, and healthcare experts and ethicists are invited to respond to its often provocative provisions. Itself structured as a dialogue, the book is an excellent way to start or to continue serious discussion about the allocation of ICU healthcare resources now and in the years ahead.

The critical care management of patients with life-threatening neurological conditions requires the ability to treat neurological injuries, manage medical complications and perform invasive procedures whilst balancing the management of the brain and the body. The Oxford Textbook of Neurocritical Care provides an authoritative and up-to-date summary of the scientific basis, clinical techniques and management guidelines in this exciting clinical discipline. This highly authoritative textbook is conceptually divided into three sections. Section 1 provides an accessible guide to the general principles of neurophysiology and neuropharmacology, cardiorespiratory support, management of fluids and intracranial hypertension. Section 2 outlines the use of multiple monitoring modes which are crucial to diagnosis and management. Section 3 covers the management of the major pathologies encountered during neurointensive care with an emphasis on evidence-based practice or consensus guidelines to provide a cohesive and definitive clinical resource. Authored by an international team of expert practitioners this textbook reflects world-wide practice and guidelines. This volume is published with a concurrent online version, which features access to the full content of the textbook, contains links from the references to primary research journal articles, allows full text searches, and provides access to figures and tables that can be downloaded to PowerPoint ®. Designed for consultants and trainees in neurocritical care, The Oxford Textbook of Neurocritical Care is also accessible to the general critical care physician and trainees who need a definite and authoritative resource to meet the unique needs of neurocritically ill patients.

Health care is ubiquitous in the industrialized world. Yet, every medical development, technique, and procedure impacts the environment. Green bioethics synthesizes environmental ethics and biomedical ethics, thus creating an interdisciplinary approach to sustainable health care. Notably, green bioethics addresses not the structure of environmental sustainability in health-care institutions but the sustainability of individual health-care offerings. It parallels traditional biomedical ethics by providing four principles for ethical guidance: distributive justice, resource conservation, simplicity, and ethical economics. Through these four principles, green bioethics presents a coherent framework for evaluating the sustainability of medical developments, techniques, and procedures. The future of our world may very well depend on how effectively we halt ecological destruction and conserve our resources in all areas of life. The principles of green bioethics, outlined in this book, will advance sustainability in health care.

In Environmental Ethics and Medical Reproduction, Dr. Cristina Richie uses the term "medicalized reproduction" (MR) to describe the impact of technology on human reproduction, including from pre-conception gamete retrieval, in-vitro fertilization (IVF), and birthing suites. Unlike other areas of high-carbon health care, such as organ transplantation or chemotherapy, medicalized reproduction does not treat, cure, or prevent disease. It is supported by an economized medical industry, and as such, is open for ethical scrutiny. This book considers how technology has fundamentally changed the discussion on biomedical ethics, environmental ethics, and reproductive ethics.

This Open Access book highlights the ethical issues and dilemmas that arise in the practice of public health. It is also a tool to support instruction, debate, and dialogue regarding public health ethics. Although the practice of public health has always included consideration of ethical issues, the field of public health ethics as a discipline is a relatively new and emerging area. There are few practical training resources for public health practitioners, especially resources which include discussion of realistic cases which are likely to arise in the practice of public health. This work discusses these issues on a case to case basis and helps create awareness and understanding of the ethics of public health care. The main audience for the casebook is public health practitioners, including front-line workers, field epidemiology trainers and trainees, managers, planners, and decision makers who have an interest in learning about how to integrate ethical analysis into their day to day public health practice. The casebook is also useful to schools of public health and public health students as well as to academic ethicists who can use the book to teach public health ethics and distinguish it from clinical and research ethics.
