Gynecologic cancers are an important problem in older American women. These patients may experience significant physical and mental problems that negatively impact their health-related quality of life (HRQOL). While HRQOL is becoming an increasingly important part of cancer research, relatively little is known about HRQOL in older women with gynecologic cancers. The objective of this dissertation was to characterize HRQOL in older American women with gynecologic cancer using the Surveillance, Epidemiology, and End Results – Medicare Health Outcomes Survey (SEER-MHOS) linked data resource. We identified female Medicare Advantage Organization enrollees who were at least 65 years old when diagnosed with a first primary invasive cervical, ovarian, or uterine cancer and participated in MHOS between 1998 and 2014. The three research studies in this dissertation each addressed a different aspect of HRQOL in older gynecologic cancer patients. The first study assessed the impact of depressive symptoms on HRQOL in women with gynecologic cancer versus matched cancer-free controls. We found that women with cancer were significantly more likely to screen positively for depression and that women with cancer and depressive symptoms had significantly worse HRQOL than cancer-free controls and women with no depressive symptoms. The second study evaluated the likelihood of developing functional impairments after cancer diagnosis and estimated pre- to post-diagnosis changes in HRQOL in women with cancer compared to a matched group of women without cancer. We determined that cancer patients, particularly those diagnosed at advanced stages, had significantly greater odds of functional impairment and experienced greater declines in HRQOL compared to controls. The third and final study examined the association between HRQOL, functional impairments, and depressive symptoms measured before cancer diagnosis with survival in older women with endometrial cancer. We observed that both physical and mental aspects of HRQOL were associated with survival and that women with worse HRQOL had significantly worse prognosis. Together these studies provide evidence of the significant impact that gynecologic cancer has on both physical and mental aspects of HRQOL in older women, and they underscore the need for tailored interventions to reduce pain, provide support, and prepare older women for changes in functioning and health after diagnosis with gynecologic cancer. Further, these findings demonstrate the value of HRQOL and other patient-reported outcomes in cancer research, as many physical and emotional problems cannot be measured by traditional laboratory tests. HRQOL measures can thus be used to better identify women with cancer who are at greatest risk of declining health, functional impairment, and depression. Assessment of HRQOL in cancer patients may help providers routinely monitor changes in physical and mental well-being, inform treatment decisions, and determine appropriate supportive care.

In an era of promising advances in cancer research, there are considerable and even alarming gaps in the fundamental knowledge and understanding of ovarian cancer. Researchers now know that ovarian cancer is not a single disease-several distinct subtypes exist with different origins, risk factors, genetic mutations, biological behaviors, and prognoses. However, persistent questions have impeded progress toward improving the prevention, early detection, treatment, and management of ovarian cancers. Failure to significantly improve morbidity and mortality during the past several decades is likely due to several factors, including the lack of research being performed by specific disease subtype, lack of definitive knowledge of the cell of origin and disease progression, and incomplete understanding of genetic and non-genetic risk factors. Introduction: The survival rates improvement of oncogynecologic surgeries and adjuvant therapies has led to focus on their long-term effects. Lower tract urinary symptoms (LTUS), sexual dysfunction (SD) and impaired quality of life (QoL) have been described as common complications in survivors. Disruption of paracervical autonomic fibers during hysterectomy and radiation cystitis could be main LTUS causes; while psycho-affective deterioration may play great role on SD.Our aim was to assess changes on these aspects after oncogynecological radical treatment.Methods: This prospective cohort study included oncogynecological patients (endometrial, cervical and ovarian) undergoing radical-intent treatment in our unit, March2017-June2018, classified according the received therapy. ERAS protocol (Enhanced Recovery after Surgery) was performed. Baseline features as age, surgical-obstetric history, comorbidities, emotional traits and relationship stability were considered. All patients were surveyed before and 6 months after treatment, using validated questionnaires: ICIQ-UI-SF (International-Consultation-Incontinence-Questionnaire) evaluating LTUS impact on QoL, CACV (u201cCuestionario-de-Autoevaluaciu00f3n-de-Control-Vesicalu201d) for detrusor dysfunction, ISFS (Index-of-Feminine-Sexual-Function) measuring SD risk, and SF-12 providing QoL profile.Results: 67 patients were finally included, without finding differences in baseline characteristics among the 46 ones completing both surveys and analysed. Pre-treatment, 50.7% patients reported urinary incontinence (UI), 74.6% no sexual activity and all high QoL scores. In the follow-up, an UI worsening tendency with advanced age was found independently of treatment modality, not reaching statistical significance. Mental QoL score in SF-12 reduced ~2.5 points if undergoing radiotherapy. SD did not differ from pre-treatment status.Conclusion: Urinary and sexual function, as global QoL, remained stable 6 months after oncogynecological treatment. Although LTUS is difficult to assess being common in gynecological patients baseline, special attention is recommended in older women to avoid UI worsening. Those receiving radiotherapy may benefit from mental QoL support. Further research with larger populations is needed to accurately assess the impact of radical oncogynecological treatments; probably leading to develop patient-targeted recovery programs.
