INTRODUCCIÓN Los indicadores demográficos demuestran el progresivo envejecimiento de la población mundial: esta tendencia y el hecho de sobreenvejecimiento y la gran ancianidad en la población española conllevan un aumento espectacular en las enfermedades llamadas "edad dependiente", destacando las enfermedades neurodegenerativas, especialmente la enfermedad de Alzheimer. Con respecto al tratamiento durante muchos años se ha mantenido una actitud nihilista ante la asistencia a pacientes con trastornos cognitivos, especialmente en el caso de las demencias (Fillit, 2000). Sin embargo, poco a poco, se ha ido asistiendo al nacimiento y puesta en marcha de estrategias no farmacológicas con la pretensión de mejorar o demorar el deterioro y déficit cognitivo presente en dichos pacientes, lo cual redundará sin duda en un mejor pronóstico y calidad de vida. OBJETIVOS Va a interesar: 1,- Valorar la eficacia de la terapia combinada (Fármaco y Estimulación Cognitiva) en los EA. 2,- Pretendemos potenciar los tratamientos no farmacológicos con el fin de tener un mayor abanico de posibilidades en el abordaje de las distintas fases por las que pasan los EA una vez comenzando su deterioro. MATERIAL Y MÉTODOS La población inicial la constituyen 102 individuos, de éstos participaron en el entrenamiento 88 sujetos. Se realizó una evaluación inicial y otra al final de la intervención. Las sesiones eran diarias con una duración de 3/4 de hora. El Tratamiento que se aplicaba era un Tratamiento Combinado (Fármaco [donepezilo] y Estimulación Cognitiva) éste consiste en entrenar funciones cognitivas (lenguaje, orientación, esquema corporal, reminiscencia, etc.) a través de sustrato sensitivo (música e imágenes) fundamentalmente. Las pruebas aplicadadas fueron: en el área cognitiva MMSE y ADAS-Cog, en el área emocional el GDS (Escala de Depresión Geriátrica) y el FAST para determinar el nivel clínico evolutivo de deterioro. RESULTADOS Hemos realizado análisis no paramétricos, por ajustarse mejor a las características de la muestra. La prueba realizada para observar si hay diferencias significativas en los diferentes grupos de Estudio I y II es la de Kruskal-Vallis. Los tests utilizados para medir el área cognitiva fueron el MMSE y ADAS-Cog mostrando diferencias significativas. El GDS (Escala de Depresión Geriátrica) utilizado para medir el área emocional no muestra diferencias significativas. El FAST utilizado para determinar el nivel de deterioro muestra diferencias significativas. Cabe destacar el análisis por pares de grupos que se realizó con el test de Mann-Whitney. CONCLUSIONES Los sujetos que han recibido Tratamiento Combinado (Fármaco y Estimulación Cognitiva) y los sujetos que han recibido solo Tratamiento Cognitivo mejoran significativamente en el área cognitiva frente a los que no han recibido Tratamiento y los que sólo han recibido Tratamiento Farmacológico. Los sujetos que han recibido Tratamiento Farmacológico mejoran en el área emocional. Los sujetos que han recibido Tratamiento Combinado (Fármaco y Estimulación Cognitiva) mejoran significativamente en el área emocional; con respecto a los que no han recibio ningún tratamiento.
