"Acute otitis media (AOM) is the most common reason children are prescribed antibiotics in the US. While the evidence-based practice (EBP) guidelines from the American Academy of Pediatrics and American Academy of Family Physicians provide guidance on the diagnosis and management of AOM, adherence to the guidelines remains low and the overall use of antibiotics remains high. The overprescribing of antibiotics and the use of broad-spectrum antibiotics for AOM treatment contributes to the growing problem of antimicrobial resistance. A comprehensive literature review showed that educational interventions increased provider knowledge and adherence to guidelines and reduced antibiotic use for the management of AOM. This EBP project aimed to increase the knowledge levels of providers and improve provider intent to utilize the EBP guidelines for the diagnosis and management of AOM. The target population was nurse practitioners and physician assistants working in retail clinics of a large organization in South Texas. A total of eighteen providers participated in the project. A self-developed tool was used to measure the success of the educational intervention. Participants completed a pretest, education session, and a posttest. Additional education sessions were offered to the participants and a second posttest for retention of information was done after eight weeks. The results showed that the knowledge levels of the providers increased by 43% in the first posttest and 56% in the second posttest, while the benchmark was a 15% increase. Similarly, provider intent to utilize EBP guidelines increased by 33%, more than the benchmark of 15%. The success of this EBP change project makes it appropriate for practical use in nursing practice. Based on documented studies, there is much evidence that by delaying the prescribing of oral antibiotics for acute otitis media as many as 75% of prescriptions for antibiotics could be avoided (Newson, 2009). There are very few risks involved with delayed prescribing of antibiotics for 48-72 hours for acute otitis media, when the parents are aware of signs and symptoms to watch for. Acute otitis media is responsible for 30 million outpatient appointments annually, costing the United States $2.2 to $3.4 billion annually (Mancini and Mandel, 2012). By better education the parents of young children many of these visits and the cost incurred with them could be avoided. There are also a number of alternative treatments that will be looked at to reduce or eliminate cases of acute otitis media. This study looks at one hospitals evidence based approach to educate their providers and nurses on the benefits of delayed prescribing of antibiotics, and to recognize the difference between acute otitis media and other potentially more dangerous conditions. The solution is not simple, as many providers have been prescribing oral antibiotics at the onset of ear pain and redness since the inception of penicillin. Parents expect to receive this treatment. In order to reduce the total number of oral antibiotic prescribed for acute otitis media much education is going to need to be done starting with the providers and nurses and continuing onto the parents. This is one of the areas that an alternate treatment can be beneficial because it helps a parent to feel empowered verse helpless. Limiting the usage of pacifiers to children less than ten months of age, encouraging of breast feeding, proper positioning of infants during feedings are simple changes that the nurse can educate parents on. Xylitol gum and suckers have also been shown to reduce or eliminate acute otitis media in children old enough to chew gum. Garlic oil has many properties that help acute otitis media to heal without the use of antibiotics. Most parents experience acute otitis media at least once with their child, by educating and preparing them on what to expect, they will be better prepared for when and if their child develops sudden ear pain.

Clinical practice guidelines are developed to minimize variations in treatment patterns while assisting healthcare providers in clinical decision making. The United States Agency for Health Care Policy and Research published the Otitis Media with Effusion (OME) in Young Children Clinical Practice Guideline Number 12 widely disseminated by 1996. The guideline provides information on diagnostic criteria, hearing evaluations, environmental risk factors, therapeutic interventions, and a treatment algorithm. Initial treatment for OME does not require antibiotic therapy lessening microbial drug resistance. Little documentation exists reporting the application of this guideline. This study used a descriptive quantitative design to examine adherence to this guideline by healthcare providers in a United States Air Force Medical Treatment Facility. A total of 196 medical records of children diagnosed with otitis media were audited using a checklist developed from the treatment algorithm. Twenty-three of these children had OME. Using summary statistics, the audit showed antibiotics, decongestants, antihistamines, steroids and surgery were not used as per guideline recommendations. Areas of low compliance were use of the six-week follow-up interval, documentation of environmental risk factor control counseling, and documentation of the use of pneumatic otoscopy and/or tympanometry to evaluate the tympanic membrane for OME and acute otitis media.
