In the relatively short period since Cryptosporidium was recognised as a human pathogen, and that it could be transmitted in water as well as directly between animals and people, it has been the subject of intense investigations. Its status as an opportunistic pathogen, especially in AIDS patients, and the lack of effective anti-cryptosporidial drugs have served to emphasise the public health importance of this organism. This has to some extent overshadowed the fact that Cryptosporidium is also an important pathogen of domestic animals and wildlife. The enteric protozoans Cryptosporidium spp. and Giardia duodenalis are the most common diarrhoea-causing protozoan parasites worldwide. Cryptosporidium spp. is a leading cause of diarrhoea morbidity and mortality in children younger than 5 years of age in poor-resource settings in sub-Saharan Africa and South Asia. Although infection by G. duodenalis is rarely a fatal condition, giardiasis is commonly associated with childhood growth faltering and cognitive impairment. Because of their significant socioeconomic impact, particularly in low-income countries, giardiasis and cryptosporidiosis joined the "Neglected Disease Initiative" launched by the World Health Organization in 2004. Both Cryptosporidium spp. and G. duodenalis are ubiquitous in the environment and can infect a wide range of hosts with different specificities, meaning that humans may acquire the infection via waterborne, foodborne, or zoonotic transmission. Recent advances in detection and molecular epidemiology have indicated that certain species/genotypes of Cryptosporidium and G. duodenalis have an expanded range of suitable hosts, suggesting that their transmission pathways are more intricate than previously thought, challenging our current notion of host specificities. Therefore, there is a clear need for more studies aiming at investigatingthat aim to investigate the frequency and molecular diversity of these parasites in humans, production and companion animals, and wildlife species. This information would be extremely useful to elucidate the transmission dynamics of cryptosporidiosis and giardiasis, and to investigate the exact contribution of zoonotic events to human infections.

As the human population grows-tripling in the past century while, simultaneously, quadrupling its demand for water-Earth's finite freshwater supplies are increasingly strained, and also increasingly contaminated by domestic, agricultural, and industrial wastes. Today, approximately one-third of the world's population lives in areas with scarce water resources. Nearly one billion people currently lack access to an adequate water supply, and more than twice as many lack access to basic sanitation services. It is projected that by 2025 water scarcity will affect nearly two-thirds of all people on the planet. Recognizing that water availability, water quality, and sanitation are fundamental issues underlying infectious disease emergence and spread, the Institute of Medicine held a two-day public workshop, summarized in this volume. Through invited presentations and discussions, participants explored global and local connections between water, sanitation, and health; the spectrum of water-related disease transmission processes as they inform intervention design; lessons learned from water-related disease outbreaks; vulnerabilities in water and sanitation infrastructure in both industrialized and developing countries; and opportunities to improve water and sanitation infrastructure so as to reduce the risk of water-related infectious disease.
