This book presents 100 challenging cases encountered in vascular surgery practice that were selected from the author’s vascular registry of 7,000 vascular reconstructions (endovascular and open). Chapters include difficult surgical problems of patients with aortic, visceral and peripheral arterial aneurysms, with expert management of mycotic, inflammatory and ruptured aneurysms and major complications like aorto-duodenal fistula, graft infection or inferior vena cava injury. These reconstructions are representative of the “real world” experience of vascular surgery in the United States, and the selection of cases were based on either difficulty with case selection, or complex operative or an endovascular technique. The natural history of vascular reconstructions often leads to secondary reconstructions, which are far more complex and therefore are included in significant numbers in this text. Both good and unsatisfactory results have been included and the lessons learned from the poor outcomes are emphasized. Invited commentaries from leading experts in the field have been added to a few chapters offering an alternate viewpoint in the management of patients with complex pathology. Challenging Arterial Reconstructions: 100 Clinical Cases serves as an invaluable resource to vascular surgeons that are learning or regularly performing open surgical or endovascular interventions.

This must-have manual for surgical residents and vascular surgery trainees is copiously illustrated with more than 250 original line drawings. The author presents in a step-by-step fashion the techniques for all common vascular reconstructions, basics of exposure and access, as well as invaluable technical pearls and tips. The text provides the surgical trainee with the opportunity to review and practice the necessary vascular reconstructions before entering the operating room. The illustrations and clear instructions make this a manual that is referred to again and again throughout the surgical residency.

In recent years, the growth in endovascular procedures has meant that vascular trainees may not gain sufficient experience to be competent in open vascular reconstructions following completion of their training. At the same time many senior surgeons have somewhat limited experience of endovascular techniques. This book will assist junior and senior surgeons and trainees in performing all types of vascular reconstructions. A key feature of the book is its clear illustrations, accompanied by pertinent case examples from vascular registries, so that readers can perform these operations with clear understanding of anatomical exposure and dissection, simplifying the conduct of the procedure.

When the first edition of Vascular Reconstructions was published almost twenty years ago, endovascular surgery was still in its infancy. Endovascular approaches became the preferred initial intervention for many vascular pathologies resulting in a major decrease in open vascular surgical procedures. In order to build upon changes in the management of vascular pathology over the past two decades, this second edition of Vascular Reconstructions was conceived. The first part starts with a review of commonly used vascular instruments and an overview of grafts and sutures used in vascular reconstructions. A detailed chapter on vascular anatomy and exposures is provided to serve as a quick reference before starting a vascular procedure. The remaining chapters in the first part of this book review the basic steps usually performed before and after constructing a vascular anastomosis in addition to thrombectomy and endarterectomy. The second part of this book focuses on the various methods used to conduct a vascular reconstruction, which include primary closure, closure with a patch angioplasty, end-to-end, end-to-side, and side- to-side anastomoses. The various possible modifications used are outlined. The third part of this book reviews the various adjunctive methods used when constructing the proximal or distal anastomoses of an infrainguinal bypass. The fourth part of the book reviews the various modifications that are carried out when constructing the proximal and distal anastomoses of an aortic occlusive or aneurysmal pathology. The vascular anastomoses workshop that inspired the conception of this book is included as an appendix. The fifth part focuses on endovascular therapy and imaging. Vascular Reconstructions: Anatomy, Exposures, and Techniques 2nd Edition aims to make the vascular rotation a pleasant experience for surgical residents, the vascular faculty, recent graduates embarking on conducting vascular reconstructions independently, and healthcare providers who wish to be familiar with the various steps involved in conducting a vascular procedure.

This book shows how new technologies and technical skills together with deeper understanding of pathophysiology of visceral and renal ischemia have contributed to significant improvements in the clinical outcomes in patients undergoing complex aortic repair involving splanchnic arteries. In recent years, aortic repair has expanded its borders, focusing more and more on the particularly challenging segments from which critical branches originate. Optimal results in this area are obtained through a multidisciplinary approach based on crucial elements such as sophisticated imaging techniques, advanced anesthesiological and pharmacological strategies, as well as updated surgical techniques and devices able to reduce ischemic injuries. The book presents problems tackled from different perspectives, by analyzing the main technical aspects of the surgical technique but also functional and metabolic issues. It also discusses concepts such as ischemic preconditioning and ischemia-reperfusion mechanisms, as well as innovative investigational approaches. In the “endovascular era”, it highlights the most recent updates in complex endovascular repair involving renal and visceral arteries, providing both practical tips and tricks and outcomes analyses. The volume also addresses specific, rarely reported problems in vascular surgery, including issues in renal transplant, oncologic surgery with involvement of splanchnic arteries and of the vena cava, and use of the deep veins in aortic surgery. This volume represents a powerful tool for both young and experienced operators who wish to approach the complex aortic pathology either by open surgery or through endovascular methods.”

Despite accurate preoperative assessment and adequate preparation, unexpected, and sometimes disastrous, situations can occur during vascular and endovascular procedures. This latest book in the European Vascular Course series provides vascular and endovascular surgeons with the skills and techniques needed to solve these unforeseen intraoperative complications. It includes thirteen chapters on aortic neck problems during open and endovascular repair, ruptured clamp sites, ureteral lesions, unintentional covering of side branches, totally calcified aorta and embolization. Accidental conditions during carotid and lower limb revascularization are also covered in six chapters and additional challenges are described in another seven chapters.
