Vols. 2-6 of the CAIB's Final Report contain appendices that provide the supporting documentation for the main text of the Final Report contained in Vol. 1, which was released on Aug. 26, 2003. These appendix materials were working documents. They contain a number of conclusions and proposed recommendations, several of which were adopted by the CAIB in Vol. 1. The other conclusions and proposed recommendations drawn in Vols. 2-6 do not necessarily reflect the views of the CAIB but are included for the record. When there is conflict, Vol. 1 takes precedence. It alone is the CAIB's official statement.

NASA commissioned the Columbia Accident Investigation Board (CAIB) to conduct a thorough review of both the technical and the organizational causes of the loss of the Space Shuttle Columbia and her crew on February 1, 2003. The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. The damage was undetected during the mission. The Columbia accident was not survivable. After the Columbia Accident Investigation Board (CAIB) investigation regarding the cause of the accident was completed, further consideration produced the question of whether there were lessons to be learned about how to improve crew survival in the future. This investigation was performed with the belief that a comprehensive, respectful investigation could provide knowledge that can protect future crews in the worldwide community of human space flight. Additionally, in the course of the investigation, several areas of research were identified that could improve our understanding of both nominal space flight and future spacecraft accidents. This report is the first comprehensive, publicly available accident investigation report addressing crew survival for a human spacecraft mishap, and it provides key information for future crew survival investigations. The results of this investigation are intended to add meaning to the sacrifice of the crew's lives by making space flight safer for all future generations.

Global economic losses due to severe weather events have grown dramatically over the past two decades. A large proportion of these losses are due to severe wind storms such as tropical cyclones and tornadoes, which can cause destruction to buildings, houses, and other infrastructure over large areas. To address the growing losses, many new large-scale and full-scale laboratories have been developed. These tools are used to examine the issues that could not be solved with the traditional tools of wind engineering including model-scale boundary layer wind tunnels, simplified standardized product tests, and other methods of analysis. This book presents state-of-the-art results from the development of the many novel approaches being used to mitigate natural disasters around the world.

CBRN and HAZMAT Incidents at Major Public Events Provides methods for planning and responding to any potential hazard at major public events, newly expanded and updated CBRN and HAZMAT Incidents at Major Public Events explains how to prepare for and react to accidental and deliberate incidents involving chemical, biological, radiological, or nuclear (CBRN) materials at any High Visibility Event (HVE). Written by a leading expert with more than 30 years of highly specialized experience in CBRN defense and security, this comprehensive guide covers general planning and preparedness, training, procurement, security methods, tools and technology, incident response, and more. The fully revised second edition incorporates current best practices, new and evolving threats, and lessons learned from major events that have occurred over the past 10 years. New chapters discuss public affairs and crisis communication, CBRN forensics and investigations, and social, behavioral, and psychological issues related to crowd behavior and CBRN responders. More than a dozen all-new practical scenarios address various incidents such as radiological attacks, pandemic illness, industrial chemical accidents, and attacks with biological warfare agents. Helps readers train and manage a multidisciplinary safety and response team, including police, fire, security, medical, military, and civil protection personnel Provides procedures for early-stage planning, building response networks, and developing assessment schemes and training exercises Covers all key areas of incident response, such as initial response, detection and identification, threat assessment, law enforcement and military support, and consequence management Explains the operational environment and unique challenges of major CBRN/HAZMAT events CBRN and HAZMAT Incidents at Major Public Events: Planning and Response, Second Edition is an indispensable resource for leaders, managers, trainers, responders, and support personnel in emergency planning, law enforcement, security, emergency medicine, public health, state and local government, and military agencies that support civil authorities.

There is growing concern globally over issues of aviation safety. Awareness of previous failures and their causes is one of the most important factors in determining risks and hazards in any new operational systems. This requires experience of accidents and failures across a broad spectrum of complex systems. Every accident occurs as a result of a chain of errors, and if one of the ""links"" making up that chain can be broken, the accident might be prevented - and becomes merely an ""incident."" If you collect detailed data from a range of ""incidents,"" relating to how they occurred, and develop a consistent method for analyzing that data, you can create a potentially valuable resource to assist in accident prevention. Serious Accidents and Human Factors proposes an original and structured approach to accident prevention. In an interesting and readable collection of accounts of major accidents, drawn mainly from the aviation industry, Masako Miyagi investigates incident reports analytically and reveals the critical information hidden therein that could avert a full-blown accident or disaster. She applies an innovative analytical technique - multidimensional analysis of incident reports (MAIR), using Quantification Method III to validate the results and focus upon individual components identified within the causal chain of events that precede an accident. She advocates wider acceptance and use of the Incident Report Analysing System, ideally administered by a neutral and independent body, to help prevent accidents not only in aviation but in relation to all complex systems, such as nuclear power plants. Serious Accidents and Human Factors offers aviation industry personnel, as well as those involved more generally with safety, risk assessment, and accident prevention in other industries, a comprehensive understanding of the accident causation chain, events contributing to that chain, and a method for identifying and eliminating causal factors in a pro-active way.
