EU regulations on the safety of medical implants-such as metal-on-metal hip replacements-must be urgently tightened in response to evidence that manufacturers are seeking approval for devices in Member States with the least stringent regulatory regimes. Much greater transparency is needed about the approval process so patients and doctors can have full confidence in the implants they are using. Manufacturers and regulators must also publish more rigorous clinical data on the safety of new implants and be subject to greater scrutiny. The clinical data requirements for high-risk medical devices to be sold on the European Market are much less stringent than for new medicines. There appears to be reliance on equivalence-similarity to an existing implant-rather than clinical investigations of the implant being approved. The Committee calls for all clinical data used in the approval of a medical implant to be published without identifying patients or clinical trial participants. For products currently on the market, such data should be published immediately. There should also be a public record of every approach from a manufacturer to any notified body in the EU so that 'forum shopping' can be identified. UK regulatory body the Medicines and Healthcare products Regulatory Agency (MHRA) is criticised for its slow reaction to reports of problems with metal-on-metal hip replacements in Australia. The withdrawal of PIP breast implants also highlights the need for frequent and unannounced spot-checks of medical implant manufacturers to identify and prevent similar problems. Alkali-Aggregate Reaction in Concrete: A World Review is unique in providing authoritative and up to date expert information on the causes and effects of Alkali-Aggregate Reaction (AAR) in concrete structures worldwide. In 1992 a first edition entitled The Alkali-Silica Reaction in Concrete, edited by Professor Narayan Swamy, was published in a first attempt to cover this concrete problem from a global perspective, but the coverage was incomplete. This completely new edition offers a fully updated and more universal coverage of the world situation concerning AAR and includes a wealth of new evidence and research information that has accumulated in the intervening years. Although there are various textbooks offering readers sections that deal with AAR deterioration and damage to concrete, no other single book brings together the views of recognised international experts in the field, and the wealth of scattered research information that is available. It provides a ‘state of the art’ review and deals authoritatively with the mechanisms of AAR, its diagnosis and how to treat concrete affected by AAR. It is illustrated by numerous actual examples from around the world, and comprises specialist contributions provided by senior engineers and scientists from many parts of the world. The book is divided into two distinct but complementary parts. The first five chapters deal with the most recent findings concerning the mechanisms involved in the reaction, methods concerning its diagnosis, testing and evaluation, together with an appraisal of current methods used in its avoidance and in the remediation of affected concrete structures. The second part is divided into eleven chapters covering each region of the world in turn. These chapters have been written by experts with specialist knowledge of AAR in the countries involved and include an authoritative appraisal of the problem and its solution as it affects concrete structures in the region. Such an authoritative compilation of information on AAR has not been attempted previously on this scale and this work is therefore an essential source for practising and research civil engineers, consultant engineers and materials scientists, as well as aggregate and cement producers, designers and concrete suppliers, especially regarding projects outside their own region.

The Royal Navy invented the fast motor torpedo boat during the First World War, and used it and other small coastal craft to great effect during the Second. This book tells the dramatic story of British coastal forces, both offensive and defensive, in both World Wars and beyond. In the Second World War, British coastal forces fought a desperate battle to control the narrow seas, particularly the Channel and the North Sea, and took the war to the coasts of German-occupied Europe, fighting where larger warships could not be risked. They also made a significant contribution to victory in the Mediterranean, but it was primarily warfare in home waters that shaped wartime British Coastal Forces and left lessons for postwar development. In this book, Norman Friedman uniquely connects the technical story of the coastal craft and their weapons and other innovations with the way they fought. In both world wars much of the technology was at the edge of what was feasible at the time. Boats incorporated considerable British innovation and also benefited from important US contributions, particularly in supplying high-powered engines during World War II. In contrast with larger warships, British coastal forces craft were essentially shaped by a few builders, and their part in the story is given full credit. They also built a large number of broadly similar craft for air-sea rescue, and for completeness these are described in an appendix. This fascinating, dramatic story is also relevant to modern naval thinkers concerned with gaining or denying access to hostile shores. The technology has changed but the underlying realities have not. This book includes an extensive account of how coastal forces supported the biggest European example of seizing a defended shore, the Normandy invasion. That was by far the largest single British coastal forces operation, demanding a wide range of innovations to make it possible. Like other books in this series, this one is based very heavily on contemporary official material, much of which has not been used previously like the extensive reports of US naval observers, who were allowed wide access to the Royal Navy as early as 1940. Combined with published memoirs, these sources offer a much more complete picture than has previously appeared of how Coastal Forces fought and of the way in which various pressures, both operational and industrial, shaped them.

This ground-breaking book takes a fresh look at potential non-litigation solutions to providing personal injury compensation. It is the first systematic comparative study of such a large number – over forty – of personal injury compensation schemes. It covers the drivers for their creation, the frameworks under which they operate, the criteria and thresholds used, the compensation offered, the claims process, statistics on throughput and costs, and analysis of financial costings. It also considers and compares the successes and failings of these schemes. Many different types of redress providers are studied. These include the comprehensive no-blame coverage offered by the New Zealand Accident Compensation Corporation; the widely used Patient, Pharmaceutical, Motor Accident and Workers Compensation Insurance systems of the Nordic states; the far smaller issue-focused schemes like the UK Thalidomide and vCJD Trusts; vaccine damage schemes that exist in many countries; as well as motor vehicle schemes from the USA. Conclusions are drawn about the functions, essential requirements, architecture, scope, operation and performance of personal injury compensation systems. The relationships between such schemes, the courts and regulators are also discussed, and both calls and need for reforms are noted. Noting the wide calls for reform of NHS medical negligence litigation within the UK, and its replacement with a no blame approach, the authors' findings outline options for future policy in this area. This major contribution builds on general shifts from courts to ADR, and from blame to no blame in regulation, and is a work that has the potential to have a major impact on the field of personal injury redress.
